Tell your clinician about previous evaluations and records you already have. Ask which information would be useful and how to share it through the clinical intake process. Keep medical records and detailed clinical history out of the introductory Google form.
Discuss participation needs before testing.
If you need particular arrangements to participate comfortably, discuss them with the practice before scheduling testing. Confirm age eligibility, evaluation scope, and availability for your situation.

Does the report guarantee a diagnosis or accommodations?
No. Conclusions depend on the findings. Schools, employers, and other receiving organizations make their own eligibility and accommodation decisions. Discuss any documentation requirements at intake.
Can I write instead of recording a video?
