Start Psychological Testing Using Your Insurance Plan

We’re excited to support you. Please follow these steps to begin the testing process.

Step 1: Ensure you have picked the right Patient Pathway here

Step 2: Complete the Pre-Screen Packet

Choose the correct packet below and set aside about 30 minutes to complete it. Please have your insurance card and a valid payment method ready.

• This allows your intake clinician to focus on your specific needs.

• Our insurance coordinator will also verify your coverage and, if needed, request prior authorization (which can take a couple of weeks).

  *Note for Pre-Op Psychological Evaluations (Surgical Patients)

  If you are a surgical patient referred for a pre-operative psychological evaluation (for example, patients referred by Dr. Paya or another surgeon), please note:

  • You do not need to complete our full pre-screen packet above. That form is for patients undergoing comprehensive psychological testing.

  • Instead, simply complete our contact form through SimplePractice to the right and be sure to indicate that your request is for a pre-op psychological evaluation.

  Our team will then contact you directly to schedule your appointment and guide you through any next steps.

Step 2: Submit Your Inquiry in Our Secure Portal

Once you’ve completed the Pre-Screen Packet:

1. Return to this page.

2. Click the button below to fill out an inquiry request in SimplePractice (step 2) , our secure portal.

Through the portal, you’ll receive all required documents and be able to message us safely.
