• If you are a surgical patient referred for a pre-operative psychological evaluation (for example, patients referred by Dr. Paya or another surgeon), please note:

  • You do not need to complete our full pre-screen packet from the Get Started page. That form is for patients undergoing comprehensive psychological testing.

  • Instead, simply complete our contact form through SimplePractice below and indicate that your request is for a pre-op psychological evaluation.

  Our team will then contact you directly to schedule your appointment and guide you through any next steps.
