Fill out our BonPsy Wellness Blueprint Signature Interest Form below. Next, click on the second button below to create your secure patient portal account. This is where you’ll complete some basic paperwork and access all communication moving forward.

Step 2: Share Your Story With Us

As part of your pre-screen, we invite you to upload a short video telling us about your concerns in your own words. This helps our team get a deeper look at what you’re experiencing and what you hope to gain from testing.

  • ⏱ Length: 5–7 minutes is perfect

  • Style: It doesn’t need to be polished, edited, or “professional,” just be yourself

  • Content: Share your main concerns, what led you to consider testing, and any goals or questions you’d like answered

  • How to upload: Use our secure Video Upload Google Form to record or attach your video directly

This personal perspective gives us another angle beyond the questionnaires and forms, so we can better understand you, not just your paperwork.

Step 3: Receive Your Virtual Consult Video

Once we’ve received your pre-screen form and portal registration, one of our licensed psychologists will review your information and record a secure, personalized video for you. In this video, we’ll explain whether testing is recommended, which types of evaluations may be appropriate, and what the next steps look like.

Step 4: Schedule Your Intake & Begin Testing

After reviewing your video consult, you’ll be able to schedule your intake session directly through our SimplePractice portal. This formal appointment begins the testing process.

What to Expect

  • Personalized 1–2 Hour Intake Session
    During this in-depth consultation, your psychologist will review your history, carefully study your records, and begin exploring your unique needs.

  • Expert Guidance from the Start
    You’ll meet with a doctoral-level psychologist (not an assistant or trainee) who brings years of experience in psychological testing and school collaboration.

  • Applied Toward Full Testing
    The intake requires a $500 deposit, which will be credited toward the comprehensive testing package ($7,200 flat fee) if you decide to move forward. Please note that this deposit is non-refundable, as our clinicians dedicate significant time preparing for your intake in advance of the actual session.

Initial Investment

  • Initial Consultation Fee: $500

  • If you move forward, this is applied to your full evaluation fee.

  • If you choose not to continue, you will still receive professional recommendations based on your intake session.

Cancellation Policy

We understand life happens. However, because your psychologist invests several hours preparing and reviewing your case before your session:

  • Cancellations made less than 48 hours in advance will incur a $150 cancellation fee.

  • Reschedules are available if requested more than 48 hours before your appointment.
